Coming Home From the Hospital

Coming home from the hospital? Let's get home ready.

A discharge often needs several things at once: a bed, a commode, a walker or wheelchair, oxygen. Tell us the date, and we work with the hospital team to line it up.

Is this you?

You might be in the right place if…

You don't need to know the name of the equipment. Tell us what's happening, and we'll help you work out the rest.

  • A discharge date is set and equipment is needed at home.
  • The hospital team mentioned a bed, walker, wheelchair or oxygen.
  • You're the caregiver and don't know where to start.
  • You're a case manager arranging equipment for a patient.

How to send a discharge order

One call and one fax. We confirm receipt and tell you right away if anything is missing.

  1. Fax the order and recent notes. Regular equipment (561) 245-5443; complex rehab (561) 861-7763. Include the face sheet and the insurance card.
  2. Call us with the discharge date. (561) 245-5444. Tell us what's needed at home and who will be there.
  3. We check coverage and line up delivery. We aim to match the homecoming, and we call the family to schedule.

Insurance

What insurance usually needs

A short summary. Your plan and your records decide; we check yours before anything is ordered.

Insurance in plain words →

Hospital bed

Original Medicare
Usually covered when criteria are met
Florida Medicaid
Usually covered, with approval first

Medicare usually needs

  • Your doctor writes down a medical reason a regular bed won't work, for example you need your head raised more than 30 degrees most of the time for a heart or lung condition, special positioning to relieve pain, or traction.
  • For a bed with electric head and foot (semi-electric): you also need frequent or immediate changes in position.

Home oxygen

Original Medicare
Usually covered when criteria are met
Florida Medicaid
Usually covered when criteria are met

Medicare usually needs

  • A test of your oxygen level shows it's low enough: usually at or below 88% (or a blood gas at or below 55), at rest, while walking, or while asleep.
  • The test is done by your doctor's office, a hospital or a lab, not by the equipment company.

Tracheostomy supplies

Original Medicare
Usually covered when criteria are met
Florida Medicaid
Usually covered when criteria are met

Medicare usually needs

  • You have a tracheostomy.
  • Supplies are usually covered up to set monthly amounts, for example care kits, and a new trach tube about every 3 months. More needs a reason in your records.

Medicare Advantage, Medicaid health plans and employer plans usually follow similar rules, often with approval first. Coverage depends on your plan, your doctor's order and your medical records. This is a plain-language summary, not a decision. We verify every case before anything is ordered. As of October 31, 2026.
