For patients and families

  • Your doctor has noted a medical reason a regular bed won't work, for example you need your head raised more than 30 degrees for a heart or lung condition, or special positioning for pain.
  • For a bed with an electric head and foot: you need frequent changes in position, or to change position right away.
  • For a trapeze bar: you need it to sit up because of a breathing condition, to change position for another medical reason, or to get in and out of bed.

You don't have to collect any of this yourself. We request it from your doctor.

For providers and case managers

  • Standard written order: patient name or Medicare number, order date, item description, quantity, practitioner name or NPI, and practitioner signature.
  • Notes describing why positioning is not possible in an ordinary bed (for example head elevation over 30° for CHF, COPD or aspiration risk, or positioning to relieve pain).
  • Semi-electric beds: the need for frequent or immediate changes in body position.
  • Accessories such as a trapeze: notes meeting the accessory's own criterion.
