Healthcare professionals
For physicians, therapists and case managers
Send a complete referral the first time. Pre-check it below, then fax it. We confirm receipt and tell you exactly what's missing, item by item.

How to refer
Four steps, and you always know what's missing.
Never send patient records through this website or by regular email. Fax only, until the secure portal opens.
- Pre-check the referral. Use the pre-check below to see what the item and plan usually need.
- Fax the order and notes. Regular equipment (561) 245-5443; complex rehab (561) 861-7763.
- We confirm receipt and review it. If anything is missing, we tell you the exact point and the rule it comes from.
- We keep you posted. Through insurance approval, delivery and follow-up.
Referral pre-check
Check a referral before you send it.
Pick the item and the patient's insurance, tick what the referral already has, and see what's missing before you fax.
Runs entirely in your browser: nothing you pick or tick is sent or stored.
Tell us what you're looking for — we carry far more than this list. The general checklist below applies to almost every item. Call (561) 245-5444 if you're not sure what the item needs, or search our equipment.
