Mask styles

Mask style is about fit and comfort, not an upgrade. Your doctor, your therapist or our team helps you find the one that seals well. Tell us if a mask isn't working for you.

For patients and families

  • You saw your doctor about sleep problems before your sleep test.
  • A sleep test (at home or in a lab) shows sleep apnea at the level Medicare requires.
  • In the first 3 months you use the machine regularly (at least 4 hours a night on most nights), and your doctor sees you again to confirm it's helping.
  • Replacement supplies follow a schedule, and you confirm you still use them.

You don't have to collect any of this yourself. We request it from your doctor.

For providers and case managers

  • Standard written order: patient name or Medicare number, order date, item description, quantity, practitioner name or NPI, and practitioner signature.
  • In-person clinical evaluation by the treating practitioner before the sleep test.
  • A qualifying sleep test (AHI or RDI ≥ 15, or 5–14 with documented symptoms or conditions).
  • Re-evaluation between days 31 and 91 documenting benefit, and adherence of ≥ 4 hours per night on 70% of nights in a 30-day period.
